To Register
- Complete Registration Form;
- Print Registration Confirmation;
- Sign forms as required;
- If necessary, attach Proof of Age;
- Attach required fees. Cheques made payable to Prospect Lake Soccer Club (post-dated to May 31 is acceptable);
- Mail form(s) and fees to:

Registration is not complete until
payment has been received by the club.
No player will be permitted to practice or play with Prospect Lake Soccer Club until properly registered for the season.
